That's because it's not called. The code you see in balloon-x.c is
experimental and should IMHO be removed. Under X, balloon helps are

I think balloon help should be cleanly implemented via device methods.
Kyle disagrees because she thinks it is useless to have to reimplement
the various stuff the frame code does, such as placement of images and
I18N chars in balloon help, which is currently possible under X. I
see her point, but I think we should remove the images support from
balloon help rather than keep the frame-based implementation (the
system tooltips look so much nicer). But Kyle is the maintainer, and
I didn't press on that.
So that's the story.

Ha, well you explain it to Jens when his stuff stops working. :)
I don't intend to argue about it. There isn't any need really.
Balloon Help isn't part of XEmacs, so I'll continue to support
it regardless of what XEmacs does. If applications want the
Windows specific tooltip feature they can reference it. If they
want my stuff, they can reference it. As long as you don't
steal my Lisp symbol names (as FSF Emacs did with the timer
stuff, the bastards (I'm smiling as I write this)), I'll be happy.

The difference is Kyle's stuff uses a Frame with the minibuffer,
modeline, menus, and scrollbars off.
balloon_help.c uses X calls to open a non-bordered window
(OverrideRedirect in X terms) that looks more like the windows tool
tips.
Kyle's stuff is more general because it can use any XEmacs features
like faces, but because it is a Frame it is heavier.
balloon_help.c can only display text, but is lighter because all it
opens is a single X window and draws into it.
BTW RMS has ask me for a GPL'd version of balloon_help.c so they can
include it in FSF Emacs.

Sorry, there are two commands in balloon-x.c, show-balloon-help and
hide-balloon-help. The intent was to attach these two calls to
enter/leave events for objects of the window like extents.
