I have added -N to patcher-default-diff-command temporarily.

I'd be very happy to hear how that should be done. I've had no luck
with only adding -N to "cvs diff" options, but have always had to "cvs
add" the file first. Otherwise, I get: "cvs server: I know nothing
about some_file".
The problem, of course is when one doesn't have commit access, the "cvs
add" can't be done.
Another interesting point is that diffing after a "cvs add", but before
committing, my cvs automagically does "diff -N" for these files,
ignoring any other diff options from the command line or ~/.cvsrc...
