Minor nit: the normal documentation has the table of contents at
the *end* of the formatted document. When printing, this isn't an
issue. However, for PDF files, having the table of contents at the end
is pretty ugly and, while you can use the commercial Adobe tools to
hand-edit the PDF file and move the TOC, a quickie workaround is to just
move the TOC to the beginning of the document (this should only result
in running LaTeX several more times). Perhaps the TOC location should
be made conditional, depending on whether or not a PDF file is being
generated?
