Let me guess: you're waving the mouse over a TeX or LaTeX document?
If so, this is a bug in preview-latex (although not one I've seen
before), and an M-x preview-report-bug would seem to be in order;
that'll give us a lot more info about preview-latex's state so we
can perhaps start working out WTF has happened to the balloon-help
extent.
