What are the permissions on the .xpm files in
<PREFIX?/lib/xemacs-21.1.10/etc/toolbar/, where <PREFIX> is replaced by
your installation prefix. Everyone, not just root, needs read
permission for those files.
If this doesn't help, I suggest following up to xemacs-beta(a)xemacs.org.
