Although I use TeX, I have used the extensions .toc, .idx, and .log for various text files in the past (and still use .log a lot); they're obvious mnemonics and the 8.3 habit is hard to break :-). A few minutes' reflection suggests that the immediate reaction---a package, eg, AUC-TeX, should do it[1]---isn't going to work very well. However what could be done easily enough (I think) is to create a Custom "Variables That There Is No Consensus Default For" group. For this kind of variable, if you wanted to add TeX extensions to the ignored extensions, you would add a checkbox widget to its Custom widget list. For efficiency, we probably would want some kind of filter to weed out duplicates (I bet lots of applications produce ignorable .toc, .log, and .idx files, for example). Well, in fact I do setq `completion-ignored-extensions' to approximately that value, and then hang a function to _add_ them on to `TeX-mode-hook'. But what I do isn't my point here; the point is that "All The Defaults Suck" (for somebody; of course I know more about which ones suck for me than I do about the ones you hate) and this is one place where I think I have a model of how to handle it with existing functionality.
