Flyspell performance problem

BTW, 
Flyspell has a performance problem on large files - the larger the
file the slower Flyspell gets.

The problem is that ispell-buffer-local-words appears to be called on
any operation when Flyspell mode is enabled.  This function is looking
for local ispell definitions in the buffer, and is called by
ispell-accept-buffer-local-defs which is then called by
flyspell-accept-buffer-local-defs which is called by flyspell-word.
