For me the best solution to pain was a different keyboard. I have a
Kinesis Ergo Advantage which moves control and alt to the middle and you
use your thumbs. They also have a keyboard with Dvorak layout on the
keycap as well, but I've never tried it.
It does wonders for the pinkies.
