This is an interesting bug. The disappearance was actually an endless cycle
of map-unmap. This fixes.

* gutter.c (output_gutter): force gutter size recalculation if
what we are trying to display won't fit.
(update_gutter_geometry): new function. A per-gutter version of
update_frame_gutter_geometry.
(update_frame_gutter_geometry): use it.
(redraw_exposed_gutter): add extra debugging output.
