There seems to be no way for users to avoid it. It is serious
especially for wide characters; the first letter of a Japanese
menu is divided into ASCII characters and the beginning one is
underlined. So, in emacs-w3m, we appended "%_ " to Japanese
menu items as a workaround.
5.5 years ago ARISAWA Akihiro wrote[1] that the first letter of
a menu is underlined by default if there's no accelerator-key
spec (i.e. leading "%_"; see menubar-item.el, etc.).
