FWIW - freeshell.org (a public access unix system) also offers dsl / dialup throughout the country.  I don't have the rates off hand for DSL, but for dial up it's ~ $7 / month.

There are quite a few

Qwest (Ethernet over Copper)
Speakeasy / Megapath / Covad (Ethernet, DSL, etc)
Cogent (T1 -> OC usually a great price on a T3 or 100Mb service depending on your building)
Airband
the list just keeps going ...

What type of access and budget are you trying to work with? Any diverse path / multiple provider scenarios or just something vanilla?
